The NAHNDAHWEH TCHIGEHGAMIG Wikwemikong Health Centre is seeking a highly qualified and motivated individual for the position of Registered Practical Nurse (RPN) with the Long Term Care/Home and Community Care Program (LTC/HCC).

 

Job Summary

The RPN will be required to work in a home care setting and see assigned clients on a regular basis. You will work collaboratively with other health care providers in providing holistic care that focuses on achieving and maintaining optimal health for clients. Hours of work are 7.5 hours per day, 37.5 hours per week including weekend and holidays on a rotating basis.

 

Qualifications

• Current standard of care certificate from the College of Nurses of Ontario

• Diploma in Registered Practical Nursing

• Experience in a homecare setting an asset

• Valid First Aid, CPR and Ontario Driver’s License

• Knowledge and understanding of First Nation Communities and their culture would be an asset.

 

This is a two (2) year contract position and continuation is subject to review.

Children’s Services Coordinator

The NAHNDAHWEH TCHIGEHGAMIG Wikwemikong Health Centre is seeking a highly qualified and motivated individual for the position of Children’s Services Coordinator within the Children’s Services Program 0 – 6. This position will report directly to the Health Services Director.

 

Job Summary

This management position is responsible for the overall operation and supervision of the Health Centre’s Children’s Services Cluster, which currently includes, but may not necessarily be limited to: Early Childhood Development services, Aboriginal Head Start (On-Reserve), Fetal Alcohol Spectrum Disorder (FASD), Maternal Child Health (MCH) and Canada Prenatal Nutrition Program (CPNP). In partnership with the Primary Health Care team, the Children’s Services Coordinator will work with the Maternal Registered Nurse and the Community Health Nursing program to maintain a model of care for Children 0 – 6 years of age.

 

Responsibilities

• Administration, management, planning and supervision of a comprehensive, wholistic and culturally relevant Children’s Services Program on behalf of the Wikwemikong Health Centre. Program and services delivered to parents/guardians/caregivers with families and children ages 0 – 6.

• Program planning and management will include case management to ensure coordination of effective program delivery of services based on best practise models to ensure positive outcomes.

• To deliver comprehensive community health programs to enable the maternal and newborn population and their families achieve and maintain an optimal level of health and development from pre-conception through pregnancy, birth and early adjustment parenting.

• Collaborate with other community services to ensure continued support for Speech Project and Spirituality and Family Project.

• Ensures linkages and partnerships are established within the community of Wikwemikong and external agencies and services as required.

• Assist families with access to services for those living with complex issues or with children who require special needs.

 

Qualifications & Skills

• Baccalaureate in Nursing from a Canadian University or College diploma in Nursing.

• A current certificate of competence with the College of Nurses in Ontario, with knowledge and understanding of College Standards of Practise.

• Strong critical thinking and decision making.

• Management and supervisory experience in project management required.

• Excellent organizational, interpersonal and communication skills.

• Willingness to undertake required orientation and training.

• Demonstrates professional responsibility and accountability.

• Ability to work respectfully in a multi-disciplinary team.

• Immunization certificate or willing to take the training.

• Current certificate of CPR and First Aid

• Computer skills: MSWord, Excel, PowerPoint and internet.

• Valid Class ‘G’ Ontario Driver’s License is required
